What is measurement-based care?

Measurement-based care involves the systematic collection of data and use of the results to drive clinical decision making at the level of the individual patient. These outcomes may be symptom-based, such as the PHQ-9; lab-based, such as Hemoglobin A1C; or functional, such as the WHO Disability Assessment Scale. A clinician or organization tracks the data over time and provides more intensive treatment to those who are not at their target goals.
