King County Health and Human Services Transformation: The Familiar Faces Initiative

The Familiar Faces initiative is systems mapping, design, and improvement work centered on creating a system of integrated care for complex health populations that can eventually benefit any user of publicly funded health services. Familiar Faces are a sentinel population defined as individuals who are frequent utilizers of the King County jail (defined as having been booked four or more times in a twelve-month period) and who also have a mental health and/or substance use condition. This population was selected as the focus of the initiative with the idea that improving the health and social outcomes of this group would result in a more meaningful understanding of the types of opportunities that the Affordable Care Act and Medicaid expansion makes possible for a community.
